You will be contacted prior to your surgery with an estimate of your copay, co-insurance and/or deductible amount. You will be expected to pay your total estimated responsibility on the day of surgery. We must also make a copy of each insurance card at the time of registration.
SELF PAY

SELF PAY – COSMETIC SURGERY
Payment in full must be received the day of surgery.
